Select treatment session if it is not your first appointment.

Select Horizon / Blue Cross Blue Shield Evaluation for first appointments or if has been more than 30 days since your last appointment.
Select treatment session if it is not your first appointment.

Select Evaluation non-medicare for first appointments or if has been more than 30 days since your last appointment.
Select treatment session non-medicare if it is not your first appointment.
